We are building a marketplace that makes it easier for physicians to refer their patients to high quality end-of-life care. Today, this is a manual process, and providers have very little information on who treats their patients well. With our application, providers can ensure their patients are cared for by a high quality hospice, seamlessly make the referral, and track patient outcomes over time.
